Passport Applications
The Kootenai County Recorder's Office is a Passport Acceptance Agency, located at at 451 Government Way in
Coeur d'Alene, Idaho. Hours of operation are 9:00 a.m. to 3:30 p.m. Monday through Friday, and 9:00 a.m. to 1:00 p.m. Saturday, except holiday weekends*.

Applications: Can be obtained at the Recorder's Office or you can log onto the National Passport website.
You will need to provide the following documentation when applying for a passport. (Unusual circumstances may require additional documentation)
EVIDENCE OF CITIZENSHIP
CERTIFIED Birth Certificate with parents names, file date, original seal and signature from issuing office. (HOSPITAL BIRTH RECORDS WILL NOT BE ACCEPTED); or
Previous U.S. Passport (preferred in place of Birth Certificate; it will be returned to you); or
(for U.S. Citizens born abroad) - Consular Report of Birth, Certificate of Birth or Certificate of Citizenship; or
(for Naturalized U.S. Citizens) - Certificate of Naturalization
PROOF OF IDENTITY
State Drivers License; or
Current U.S. Passport; or
Expired forms of ID are not accepted
Parents ID will be used for children under 16 years old
Note: Expired forms of identification, Temporary Identification or Temporary Driver's License are NOT Accepted.
PHOTOGRAPHS
Two recent identical passport photos (portrait photos are not suitable)
FEES
Separate checks or money orders made payable to the U.S. Department of State are required for each applicant
applying for the Passport Book or Passport Card. NO CASH.
One check or money order for more than 1 applicant is acceptable for the processing fee, payable to Kootenai County. Cash is acceptable for
the Processing fee.
Age 16 and older:
Passport Book: $110 check or money order only, made payable to the U.S. Department of State (plus $60 Expedite Fee if desired)
Passport Card: $30 check or money order only, made payable to U.S. Department of State (plus $60 Expedite Fee if desired)
AND
Processing fee: $25 cash, check or money order, payable to Kootenai County
Age 15 and younger:
Passport Book: $80 check or money order only, made payable to U.S. Department of State (plus $60 Expedite Fee if desired)
Passport Card: $15 check or money order only, made payable to U.S. Department of State (plus $60 Expedite Fee if desired)
AND
Processing fee: $25 cash, check or money order, payable to Kootenai County
Please Note:
All applicants must be present, including both parents for minors under the age of 16.
ALL applications must be SIGNED IN THE PRESENCE OF the Recoreder's Department Acceptance Agent. Applicants 16 years and older must
sign their own application.
Allow approximately 4-6 weeks processing time for delivery by the U.S. Department of State, unless you have paid Expedite Fees, in which
case approximately 2-3 weeks is required for delivery from the U.S. Department of State.
